Everyone knows that diet, lifestyle choices like alcohol consumption and smoking, exercise, and genetics play a role in your overall health. But experts suggest that social determinants—that is, the conditions in the environments where people are born, live, learn, work, play, worship and age—can drive up to 60% of health outcomes.

Social determinants of health (SDOH) can include access to nutritious foods, safe housing, education and job opportunities, social support systems, polluted air and water, and much more.

It’s critical for public health organizations and community partners in the education, transportation and housing industries to work together to address SDOH.

In this infographic, we look at three benefits of addressing SDOH, explore examples of services and supports that can be covered under Medicaid and the Children’s Health Insurance Program (CHIP) to address SDOH, and highlight how six states are engaging in efforts and initiatives to improve SDOH for disadvantaged individuals and families. Plus, we share six strategies your organization needs to consider to understand and address SDOH.
